1) Scope

This Privacy Policy explains how we collect, use, disclose, and protect information when you visit our website, schedule an appointment, complete forms, communicate with us (including by SMS/text), or receive patient services at LC Wellness Center.

HIPAA Note: When we create, receive, maintain, or transmit Protected Health Information (PHI) in our role as a health care provider, that information is governed by the Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices (NPP) (see Section 10). This Privacy Policy supplements—does not replace—our NPP.

2) Information We Collect

PHI (covered by HIPAA/NPP): Information related to your past, present, or future physical/mental health or condition; treatment; and payment for health care, including health history, treatment records, imaging, and insurance details.

Personal Data (non-PHI): Name, email, phone number, mailing address, communication preferences, website activity (cookies, IP address, device/browser data), and marketing opt-ins (e.g., email/SMS).

Appointment & Billing: Requested services, preferred dates/times, payment method metadata (we do not store full card numbers on our servers if using a PCI-compliant processor).

Website & Analytics: Cookies, pixels, and similar technologies for functionality, security, and performance/analytics (see Section 9).

3) How We Use Information

Provide and coordinate chiropractic services and operations

Schedule and confirm appointments; provide patient support

Process payments and insurance claims (as applicable)

Communicate about your care, clinic updates, wellness resources, and (if opted-in) marketing/SMS

Secure our systems, prevent fraud, and comply with laws

4) SMS Messaging & Required Language

We will not share your opt-in to an SMS campaign with any third party for purposes unrelated to providing you with the services of that campaign. We may share your Personal Data, including your SMS opt-in or consent status, with third parties that help us provide our messaging services, including but not limited to platform providers, phone companies, and any other vendors who assist us in the delivery of text messages. All of the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

5) How We Share Information

We may disclose information as follows:

For Treatment, Payment, and Health Care Operations (PHI): As permitted/required by HIPAA (see NPP).

Vendors/Processors: With service providers under contracts that restrict use to our instructions (e.g., EHR, appointment, billing, analytics, email/SMS platforms, web hosting, security).

Legal/Compliance: To comply with law, court orders, law enforcement, or to protect rights, safety, and security.

Business Transfers: In a merger, acquisition, or asset sale, subject to applicable law and HIPAA.

We do not sell Personal Data or PHI.

Email/SMS Marketing: You can opt-in/out at any time. SMS: reply STOP to unsubscribe; HELP for help.

Cookies: Control via your browser settings and consent tools.

7) Data Security

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the data, including HIPAA-aligned controls for PHI and reasonable industry safeguards for non-PHI. No system is 100% secure.

8) Data Retention

We retain records as required by law and professional standards. Marketing and website data are kept as needed for the purposes described or until you request deletion where applicable by law.

9) Cookies, Analytics, and Tracking

We may use first-party and third-party cookies/pixels for site functionality, performance analytics, and (if you consent) remarketing. You can manage preferences through your browser and consent banners.

11) Children’s Privacy

Our services are intended for individuals seeking chiropractic care. We do not knowingly collect Personal Data from children under 13 online without appropriate consent under applicable law.

Our services are offered in the U.S. If you access from outside the U.S., you consent to processing in the U.S., where laws may differ from your jurisdiction.

13) Changes to This Policy

We may update this Policy. The “Effective Date” reflects the latest version. Material changes will be posted on our website.
